Overview
Life Helpers Initiative (LHI) is seeking a Referral Nurse to strengthen the health referral system for vulnerable women and children within a UNOCHA-funded humanitarian project in Yobe State.
Key Responsibilities
- Facilitate timely and appropriate referral of patients to health facilities.
- Coordinate with community structures and health providers for case identification and referral.
- Maintain accurate documentation of all referred cases and follow-up actions.
- Track referred patients to ensure continuity of care and treatment completion.
- Support basic clinical care services where necessary under approved guidelines.
- Strengthen linkage between communities and health facilities.
- Participate in case management activities, especially for malnutrition and maternal/child health cases.
- Conduct follow-up visits and ensure proper reporting of patient outcomes.
- Collaborate with project teams to improve referral efficiency and service delivery.
- Contribute to health education and awareness within communities.
Requirements
- Bachelor’s Degree or Diploma in Nursing (BSc Nursing, RN, or equivalent) with a valid license from the Nursing and Midwifery Council of Nigeria.
- 2–3 years of clinical nursing experience, with preference for experience in primary healthcare, maternal and child health, or nutrition programs.
- Prior NGO or humanitarian experience is an advantage, especially in Northern Nigeria.
- Strong knowledge of referral systems and case management, good documentation and reporting skills, proficiency in Microsoft Office, and strong communication skills.
- Familiarity with Hausa and/or Kanuri languages is a plus.
